How To Send A Reference Form

To send the reference form to your pastor, priest or other Christian leader follow these instructions.

• have his or her email address handy

• View the form at:
  http://www.thoughts-about-god.com/mentor/pastor_ref_form.html

• Send him/her an email explaining why you are contacting them and include a link to the form at:
  http://www.thoughts-about-god.com/mentor/pastor_ref_form.html

When they fill out the form and hit 'Submit' the information will automatically be forwarded to our mentor co-ordinator.  It is not necessary to print it and send it by post.
